Jail Pre-Release Discharge Planning and Referral
TASC, with funding from the Michael Reese Health Trust, in conjunction with the Cook County Sheriff’s Office and Cermak Health Services (the healthcare provider inside the jail), is working to create linkage infrastructure for persons with serious mental illnesses who are leaving the Cook County Jail. Building on a six-month detailed planning process, and utilizing the new healthcare coverage options made available through the ACA, the partners are implementing a pre-release discharge planning and post-release follow-up mechanism for people leaving the jail. The initial demonstration program, staffed by TASC, focuses on discharge planning and healthcare referral and linkage for people with serious mental illness leaving the jail. The program will expand over time to also reach people with severe substance use disorders and chronic medical conditions.
