The Justice & Health Initiative (Cook County, Illinois)
Through the support of The Chicago Community Trust, TASC is facilitating a health reform strategy and implementation process on behalf of the Presiding Judge of the Cook County Criminal Courts, bringing together leaders from the justice and health systems and state agencies with responsibility for health reform implementation. This planning process creates a platform for partners to identify problems, solutions, and new ideas regarding healthcare reform. Through this process, partners have been able to successfully implement several pilot projects, including a process for detainees to receive application assistance into CountyCare. Additional strategies focus on addressing continuity of care between jail and community and on integrating Medicaid managed care into services accessed through the courts.
